GLP-1 Medications and the Missing Metabolic Evaluation

If you've been told your bloodwork is "normal" but you don't feel like it, this post is for you. I'll walk through why a standard annual panel misses early metabolic dysfunction, what a full metabolic workup actually includes, and why the timing of that workup matters more than most patients realize. This is written from my own clinical practice as a physician who is also board certified in obesity medicine, which means metabolic evaluation isn't an add-on to what I do. It's the foundation of it.
Why "Normal" Labs Don't Mean a Healthy Metabolism
I hear a version of the same sentence almost every week: "My doctor said everything looks fine." Then the patient describes fatigue that sleep doesn't fix, weight that won't move despite real effort, cravings that feel biological rather than behavioral, and a general sense that something in their body has shifted.
Here's the problem. Standard reference ranges are built from population averages, and the population is not metabolically healthy. Nearly four in ten American adults meet criteria for metabolic syndrome, a cluster of risk factors that includes abdominal obesity, elevated blood pressure, and abnormal blood sugar and lipid markers, according to a national analysis of NHANES data covering 2013 through 2023. When "normal" is calculated against a population where metabolic dysfunction is this common, a lab result can sit inside the reference range and still represent a body that is already struggling.

What the Standard Annual Panel Was Never Designed to Catch
A typical annual physical usually includes a fasting glucose, a basic lipid panel, and maybe a hemoglobin A1c if a physician has a reason to look closer. That panel is useful, but it was built to catch disease that has already arrived, not the metabolic drift that happens for years before a diagnosis appears.
Consider insulin resistance. It can be actively progressing for a decade before fasting glucose or A1c move out of range, because the body compensates by producing more insulin to keep blood sugar in check. A fasting glucose test alone will not show you that compensation is happening. It will only show you the moment compensation starts to fail.
The scale of the problem this creates nationally is significant. The CDC estimates that 115.2 million American adults have prediabetes, and roughly 8 in 10 of them don't know it. That is not a small blind spot. It is the majority of an already enormous group of people whose bodies are sending signals that a standard panel isn't built to interpret.
The U.S. Preventive Services Task Force now recommends prediabetes and type 2 diabetes screening starting at age 35 for adults with overweight or obesity, a change made specifically because the previous screening age missed roughly half of the people who actually had prediabetes or undiagnosed diabetes. That's a meaningful acknowledgment from a national body that the old approach to catching this early was not catching it early enough.
What a Real Metabolic Workup Actually Includes
When a patient comes to me suspecting something metabolic is wrong, here is the kind of evaluation I run, beyond the standard annual panel:
Fasting insulin, not just fasting glucose. This is often the single most revealing addition. Elevated fasting insulin alongside normal glucose is frequently the earliest visible sign of insulin resistance.
A calculated insulin resistance estimate, using fasting glucose and insulin together, to get a clearer picture than either number provides alone.
A full lipid panel with particle-level detail where indicated, since standard cholesterol numbers can look reassuring while underlying cardiovascular risk is rising.
Inflammatory markers, because chronic low-grade inflammation is closely tied to metabolic dysfunction and is not part of a standard annual panel.
Thyroid function, since thyroid dysfunction can mimic or worsen metabolic symptoms and is frequently underevaluated in a rushed visit.
Body composition analysis, not just weight or BMI, since two people at the same weight can have very different amounts of visceral fat driving very different levels of metabolic risk.
A detailed history of symptoms, sleep, and family history, because metabolic disease has a strong hereditary component and a strong behavioral one, and both matter for interpreting the labs correctly.
None of this requires exotic testing. It requires time, a reason to look closer, and a physician who has both.
What This Looks Like in Practice
I think of a patient, a woman in her late 40s, who came to me after years of being told her labs were fine. Her fasting glucose had always been normal. Her cholesterol was borderline but nothing her previous physician flagged as urgent. She was exhausted, gaining weight around her midsection despite no real change in her habits, and increasingly frustrated at being told this was just aging.
Her fasting insulin told a different story than her fasting glucose did. Once we had the fuller picture, including body composition data that showed a shift toward visceral fat, we had an actual target to work with instead of a vague instruction to eat less and move more.
"Patients are rarely wrong about their own bodies," I tell my patients often. "They are usually just undertested." That sentence describes most of what I do in this part of my practice.
Who Should Be Asking for This
You don't need a diagnosis already in hand to ask for a full metabolic evaluation. Consider it if any of the following describe you:
You've been told your labs are normal, but you don't feel like it
You're gaining weight, particularly around your midsection, despite consistent effort
You have a family history of type 2 diabetes, heart disease, or metabolic syndrome
You're over 35 and carry excess weight, which now meets the threshold for USPSTF-recommended screening
You've tried a telehealth weight loss program or GLP-1 prescription without ever having a full metabolic workup first
Your energy, sleep, or mood has shifted in ways that don't have an obvious explanation
If several of these sound familiar, the conversation worth having with a physician is not "what diet should I try." It's "what is my metabolism actually doing, and what does the full data show."

Getting the Full Picture Starts With One Conversation
The reason this workup gets skipped so often isn't that it's complicated. It's that it requires time most primary care visits don't have. A rushed ten-minute appointment doesn't leave room to order fasting insulin, review body composition, and sit with a patient long enough to connect the dots between symptoms that seem unrelated.
